donate using a check, please tap or
click here to download the Donation Form, print it and fill it
out.
If you are an SRC resident, then you can also find donation forms
and envelopes in the IL and AL cubby areas and in the Reception Area
of the Health Care Center. Please deposit your donation form and
donation in one of the marked boxes in these areas.
If you are not a resident then please mail the donation form with
your donation to

Please make checks payable to SRC Living Legacy Foundation.
If you wish to donate directly from a financial institution such as
an IRA or an investment fund, then please ask the institution to
mail your donation to the above address for the benefit of SRC
Living Legacy Foundation.
When you donate, please consider whether you wish your donation
to be designated for resident assistance or for education, or not to
be specifically designated. The donation form has check boxes for
you to indicate your wish. However, if you are donating using the
Donate button or directly from a financial institution such as an
IRA or investment fund, then please make sure you include your wish
when you fill out the appropriate form.
The SRC Living Legacy Foundation is a Section 501(c)(3)
nonprofit organization and your gift may qualify as a charitable
deduction for federal income tax purposes. Our Tax ID is
84-3432820. We will send you a tax receipt letter in the
January of the year following the year in which we received your
donation (except for donations from financial institutions who
provide tax receipt letters).

prefer to remain anonymous, please indicate this clearly with your

Planned Giving
Here are some of the ways in which you can leave a legacy to the SRC
Living Legacy Foundation:
- Name the SRC Living Legacy Foundation as a beneficiary of your
remaining assets in your will or living trust.
- Commit a percentage of assets remaining in your living trust
after your lifetime.
- Name the SRC Living Legacy Foundation as a beneficiary of a
portion of your IRA or qualified retirement plan.
- Name the SRC Living Legacy Foundation as a beneficiary of a
portion of a life insurance policy.
